Subject:	Bargaining Units 12 and 13--Death Benefit



	Members of the International Union of Operating Engineers (IUOE) 
employed by the State of California in Bargaining Units 12 and 13 
are covered by life insurance provided by IUOE by virtue of their 
membership.

	Although IUOE periodically advises their members of this benefit, 
often times the member fails to notify the beneficiary or the 
beneficiary fails to contact the insurance plan administrators upon 
the death of a covered member.

	To ensure that the listed beneficiary receives this benefit on a 
timely basis, IUOE has requested that immediately upon death a 
notice be provided to the insurance plan administrators.

	Consequently, upon the death of a Unit 12 or 13 member, the 
departmental personnel office should notify:
