Subject:   Industrial Disability Leave with Supplementation (IDL)



            Attached is a revised copy of the new Industrial Disability Leave with
            Supplementation Benefits Information and Option Selection Form
            (STD 618-S).  This form will replace the version that was sent out with
            PML 92-122.  It was revised based on the feedback that was received from
            all of the Workers' Compensation Specialists and Personnel Services
            Specialists who attended training on the new IDL/S benefit in December
            1992.  This form incorporates all of the suggested changes.  We
            appreciate all of the support and suggestions that we have received
            relating to implementation of this new benefit.
