This brochure has been developed in accordance with Section XI New Program Development and Process Improvement (B)(2) of the new Master Agreement. It is recommended that supervisors or departmental representatives provide this brochure to the injured worker at the time they are given the Employee Claim for Workers' Compensation Benefits form (SCIF 3301). Prior to providing the brochure to the injured worker, the supervisor or departmental representative must complete the contact information section at the bottom of page two. Space is provided at the bottom of page two for each department to insert the name of its designated representative and the number for the appropriate State Compensation Insurance Fund (SCIF) office. Please note that this brochure is intended to complement the existing State Employees' Guide to Workers' Compensation pamphlet (SCIF 13730). To obtain the SCIF 13730, call the SCIF forms mailbox at (916)567-7526 and leave your name, contact number, department's name, mailing address, and the name and/or form number you are requesting. SCIF provides the SCIF 13730 at no charge to your department. This brochure is designed to answer the most common questions asked by an injured worker. The intent behind the development of this brochure is to explain the workers' compensation system to the injured worker at the onset of the claims process. Ideally, providing this information to the injured worker will help mitigate or reduce early litigation rates, which often occur because injured workers do not understand the workers' compensation system.
