  Many State employees completed Employee Action Requests 
  (EARs) indicating that their home address was to be 
  withheld from their exclusive representative.  The Unit 12, 
  13, 15, 17 and 20 collective bargaining agreements obligate 
  the State to send a letter to these individuals so they 
  understand their addresses are presently being withheld, 
  and to notify them that they may elect to have their 
  addresses released in the future.  
  
  Attached is a copy of the letter being mailed to non-law 
  enforcement employees in Units 12, 13, 15, 17 and 20 who 
  (on their Employee Action Request form) asked that their 
  home address be withheld.  It provides these employees with 
  an opportunity to remove the withhold so the State 
  Controllers Office can provide their home address to their 
  exclusive representative.  Employees electing to remove the 
  withhold so their home address is released must complete 
  the attached form and return it directly to their 
  departmental Personnel Office.  Personnel Offices will be 
  responsible for entering the change into the State 
  Controllers Office computer system.

  The purpose of this memorandum is to provide you with an opportunity 
  to have your home address released to the labor organization that is 
  the exclusive representative for your collective bargaining unit.
  
  At the time you completed an "Employee Action Request" form you asked 
  to have your home address remain confidential.  As a result the State 
  of California has not released your home address to the labor 
  organization which represents you.  You may change your decision now 
  or at any time in the future so your labor organization can 
  communicate directly with you at home. 
  
  If you choose to release your home address to your labor organization 
  you may do so by completing the following information and returning 
  this memorandum directly to your departmental Personnel Office.  DO 
  NOT return this memorandum to the Department of Personnel 
  Administration.
  
  PRIVACY NOTICE 
  
  The Information Practices Act of 1977 (California Civil Code Section 
  1798.17) and the Federal Privacy Act (5 USC 552a, subd. (e)(3)) 
  require that this notice be provided when collecting personal 
  information from individuals.  Information requested on this form, 
  which includes your social security number, is used for purposes of 
  identification and address release/withholding.  Furnishing the 
  requested information on this form is mandatory.  Failure to provide 
  the mandatory information may result in your address release request 
  not being processed, or not being processed correctly.
  
  Legal references authorizing the maintenance of this information 
  include:  Federal Internal Revenue Code (26 USC Sections 3402(a), 
  6011, 6051 and 6109) and the regulations thereto; California 
  Government Code section 12470-12479 and 16391-16395; delegated 
  authority from the State Personnel Board; and delegated authority from 
  the Trustees of the California State University.
  
  Employees have the right to review their own personal information 
  maintained by the State Controller's Office unless access is denied by
