During open enrollment, you can enroll in a dental plan, change plans, add or delete dependents from your coverage, or cancel your enrollment. Check with your personnel office for details and forms. You don't need to take any action if you want to keep the same plan and coverage.
Changes you make during the 2004 open enrollment take effect Jan. 1, 2005.
