Open Enrollment 2007 - Retirees and Annuitants - Making Dental Plan Changes by Phone

Phone requests must be made to CalPERS. Representatives are available Monday through Friday, 8:00 a.m. - 5:00 p.m. Pacific Standard Time, through October 12, 2007. When you call, be prepared to provide the information listed below under "Required Information for Open Enrollment Requests."

Required Information for Open Enrollment Requests

New enrollments for dental plan changes:

1. Your name and Social Security Number

2. Your date of birth

3. Name of the plan you wish to enroll in or change to

4. If changing plans, the name of your current plan

5. Your home phone number (optional)

Adding dependents to your dental plan:

1. Your name, date of birth, and Social Security Number

2. Name, Social Security Number, and birth date of the dependent(s) being added

3. If you are adding a spouse, a copy of the marriage certificate

4. If you are adding a domestic partner, a copy of filing documents

5. If adding a dependent(s), his or her relationship to you

6. If the dependent is not a natural child or stepchild, include verification of dependency (affidavit, adoption, custody record)

Deleting dependents from your dental plan:

1. Your name, date of birth, and Social Security Number

2. Name, Social Security Number, and birth date of the dependent(s) being deleted

3. If deleting a dependent, his or her relationship to you
