As a State of California retiree and/or annuitant, you are eligible to enroll in the State Retiree Vision Program, which is being offered through the Vision Service Plan (VSP). The Retiree Vision Program provides vision care coverage for you and your eligible dependents.
The Open enrollment for the 2007 open enrollment period is September 17, 2007 through October 12, 2007. If you enroll, your election is effective January 1, 2008 through December 31, 2008 (12 months).
If you don't enroll by October 12, 2007, your next opportunity to enroll in eyecare coverage will be the fall of 2008 with a January 2009 effective date.
There are 3 ways to enroll:
- Visit VSP at www.vsp.com/go/stateofca and complete the online enrollment form; or,
- Call VSP at 1-800-877-7195 and speak to a Member Services Representative; or,
- Complete and mail the VSP Brochure/Enrollment Form.
If you are currently enrolled in the Retiree Vision Program you don't need to take any action at this time. Your coverage will automatically continue.
